“Gum inflammation – gingivitis – and tooth support tissue damage – periodontitis – affects 90% of people in milder or heavier form, moreover, a large proportion of patients already have serious loss of support tissue,” explains Inguna Grīnvalde, head of periodontology department at Rīga Stradiņš University Institute of Stomatology.
She resembles 10 important gum health commandments that anyone who still has teeth in their mouths should know.
“If a caurs tooth hurts and sooner or later a patient visits a dentist, then gum inflammation usually does not cause pain and is sometimes overlooked for years until serious connective tissue breakdown and bone tissue breakdown develop,” the specialist warns. Consequently, the patient is sometimes surprised when a visit to the dentist reveals that a significant portion of the tooth support tissue has already been lost as a result of the spread of the inflammatory process.
Gum diseases have been progressing for years and often without specific complaints, but bleeding gums are the first signal of inflammation. Note that gum bleeding is not the norm! Gums don’t have to bleed, brushing their teeth or eating. However, gingivitis, or gum inflammation, is usually completely cusable as long as treatment is started in time.
About 300 different types of bacteria present in the oral cavity are currently known. If microbes replicate in the mouth, inflammation develops. the presence of specific microbes called periodont pathogens most affects the spread of inflammation deeper into the tissue, 1 explains. Green Board. These microbes breed in an anaerobic environment where there is no oxygen – at the depth of the microbial layer below the gum edge. As a result, the attachment of the gums to the tooth drops lower and begins to form a pocket of gums in which persistent inflammation develops.
Periodontitis is an inflammation of the tooth support tissue that binds to the degradation of connective tissue and bone tissue. This is not a reversible process as it is impossible to fully restore the lost. However, treatment can successfully remove gum inflammation and rejuvenate the support tissue of the tooth, preventing further periodontitis development and loss of the supporting bone.
‘To do this, the patient will have to clean their teeth thoroughly and correctly after the active treatment phase of periodontitis for the rest of their life, preventing accumulation of microbial bypass. Visits to a periodontologist or dentist should also be regularly scheduled to prevent recurrence and exacerbation of periodontitis.’
“There is a belief that only older people suffer from periodontitis, but in fact it is also found in young people, sometimes even adolescents,” says I.Greenboard.
As the loss of connective tissue and bone progresses, tooth mobility also appears – they move, become seemingly longer, and gaps form between teeth. The inflammation process is carried out with periodic exacerbations. Two forms of periodontitis are released: chronic, which develops relatively slowly, and aggressive periodontitis, which progresses rapidly and can quickly lead to tooth loss.
In order to ascertain the health of the gums and to detect problems in a timely manner, a physician or hygienist shall determine the depth of the gum groove or abnormal pocket of gums by means of a special periodontal probe, while also detecting haemorrhage of the gums or excretion of pus by probe. Diagnostic X-rays are also underway to assess bone levels.
‘If the gums bleed during probe and the depth of the pockets exceeds the norm, treatment should be initiated in good time. Unfortunately, sometimes even in young patients the problem is so blown in that it is necessary to start assessing how many of the patient’s teeth will be saved at all …” says the medic with regret.
Relatively often, patients require periodontitis surgical treatment that helps eliminate inflammation, makes it easier for teeth to be cleaned in hard-to-access areas, and stimulates support tissue regeneration, thus improving future predictions for victims “teeth. However, the operation is unfortunately unable to ensure that the lost bone and connective tissue are fully recovered.
Proper brushing of teeth, which has become a habit, can prevent the development of gum inflammation in time. Often, however, there is a lack of time and motivation. ‘Typical error is people’s desire to clean the front surfaces of their teeth as best they can, which is better visible. They are then also rubbed using hard toothbrushes and abrasive toothpastes, using strong horizontal or circular movements. The result is often a gum retreat, or recession. Tooth hypersensitivity and cosmetic defects are often seen when exposed to the neck of teeth,” explains the specialist.
On the other hand, the application, which also causes inflammation, often persists between teeth and on the inner surfaces of teeth after cleaning.
A dentist or hygienist should teach their patients the right technique for cleaning teeth. ‘Proper tooth brushing movements are milking, pointing from gum to tooth, slightly rotating and vibrating in the direction from gum to tooth using a soft toothbrush. A good effect can also be achieved with an electronic toothbrush because it performs vibrantly rotating movements,” explains L. Green Board. The biting surfaces should be cleaned by horizontal movements and the surface of the tongue should not be forgotten.
If the contact between teeth is tight, dental floss should be used to clean the interdental area. If there are gaps between teeth, special single-bundled brushes should be used to clean the interdental gaps. For patients with prostheses, crowns, bridges, implants and braces, the correct cleaning agents will be recommended and shown by a dentist or hygienist.
A mum or dad should help the child clean their teeth while the baby’s hand is not as agile yet.
‘It should be noted that we consume a lot of refined products glued to our teeth today, so brushing teeth with eating carrots unfortunately cannot be replaced. However, all substances necessary for the body as a whole will also benefit gum health,” explains the specialist.
Smoking is one of the most serious and common factors contributing to periodontitis, which, along with poor hygiene, greatly increases the build-up of circumstance. “When instituting periodontitis treatment, smokers should be aware that the chances of treating the disease will be twice as low,” warns L. Green Board.
On the other hand, stabilising blood sugar levels first is crucial for the initiation of elective treatment of periodontitis diseases in diabetic patients. If this is achieved, the treatment results are good. However, studies show diabetes patients have greater loss of teeth and support tissue.
However, smoking contributes far more to the development of gum diseases than diabetes. If a person smokes 10 cigarettes a day, gum disease progresses twice as quickly.
10-12% of people are highly susceptible to gum diseases
With the proliferation of microorganisms in the mouth, this group of people dramatically changes the body’s immune responses and the processes of gum inflammation develop very quickly. They suffer from gum problems early on, which only develop as the years go by.
10-12% of people are not susceptible to gum disease
It is said of these people that his grandfather did not clean his teeth all his life, but he died at a gray age with all his teeth, and his own mouth has pearls, even without strict hygiene. These are non-susceptible organisms whose microflora does not contain or contains periodont pathogens in very small quantities. These people are unsusceptible to microbial-induced gum diseases.
76-80% of people depend on gum response
of the amount of microbes in our mouths, most often there is a relationship – the more we accumulate circumstance, the more inflammation develops. The better we deal with circumvention, the healthier our gums. «inflammation is a protective effect on the microbe – aggressor. Gums are a number of defensive mechanisms that try to fight this aggressor, and for a while it also succeeds. While the body has strong immunity and there is a balance between aggressor and the body’s defensive capabilities, one doesn’t even feel that a certain phase of inflammation has developed in the gums. In the event of weakening immunity or acute illness, gum problems worsen.
